I have read of many cases when steering failure is due to wore ropes jumping off quadrants and pulleys.
If you look at a steering quadrant, you see that much of the rope never leaves the quadrant. We put hose clamps on that part, so that the wire cannot jump

We also put a small fabrication on the fixed pulleys that prevent the wire jumping off. All of this took about 3 hours and cost under $10.
