ours because a friend went over the side on his boat when a swedge let go. He spent half an hour hanging by his harness beating on the side of the boat until his wife woke up. That's a point made for using a harness and another for having a "Watch Commander" installed. I made sure every turn buckle, threaded ring and stud had locknuts. In '07, the first year of our last cruse into Mexico, three people, that I know of, went over the side. All lifeline issues. Thankfully, all three were recovered.
