His tapers on a 1" thick hole were only 3 or 4 " at the most..They should be more like 16"-18" minimum in my opinion. He made a plug with not nearly enough taper to spread the load. He needs to remember this is purely a mechanical connection on those repairs. It isn't like laying in fresh glass.
