The fault wasn't enough to trip the breaker
In Response To: The water heater element was shorted to ground inside the element. ()
Since the fault was into the water there must have been enough resistance to not trip the breaker, but it was enough to drive the ground potential to 75 volts. As Tom said, that must be rare.
