Carefully using an Electric Lewmar 66 or one of the large manual winches. When using the electric winch, you have to watch the person in the chair constantly in case they get "hung-up" in some the rigging or spreaders (or, God forbide, the winch switch fails). We use rope clutches as a safety too.
Using a windlass seems more tricky -- the lead for the captstan is tougher; and, you have to tail it a lot more agressively. I have tried this once (with someone on the windlass), it scared me a LOT. If you use a safety line with (de)ascenders, it would be easier.
I just prefer the main halyard, whenever possible.
