I have an Icom 710RT and use a head set all the time.

The 710RT has the plug on the remote head coming out the back on a short lead. I'm not sure where the plug is on a standard unit is but you'll find it. I use two different headsets. One has the standard large male plug and I use a over the counter reducer which allows me to use it instead of the newer one with the mini plug. I find using the head set is much better then trying to hear with the built in speaker so I use them all the time and hardly ever use the speaker. Using the head set also lets my wife go about her business without having to listen to all the check-ins and general traffic. Using them doesn't block out all of the engine noise but knocks it down enough to significantly help with picking up soft stations. I find that turning off the usual offenders such as the refrigeration units, the chart plotter, RADAR, fluorescent lights and fans seem to help. Of course, shutting down the engine helps to but sometimes that's not practical. Shutting down the engine at dinner time while my wife is cooking and having the boat roll all over the place is a sure way to get her to stop making me dinner.
