Maintenance issues? 80% chance it was.
In Response To: I missed photo day, but here ya go. This ain't good.

() I replaced all our chainplates but not the forestay fixture (whatever it's called). Always thought it was a bad idea not to complete the job leaving that part untouched. The last surveyor (2012) I had done in Brunswick recommended I do it.
