It's not designed to be tied up to. Always attach your bridle to the bottom of the ball.

That wasn't a hardware failure..it was an operator failure..
It was neglect!
Monel is more corrosion resistant than stainless particularly in the absence of oxygen

Never? If the chain comes through to the top it's OK
Here in Onset, only the chain-through-the-ball is allowed.

What actually happened to that hardware ?
Yes thread so corroded that the pin just slipped out
I don't think you can blame the wire ties here.
It looks as though that shackle has lost a third of it's section where it met the swivel, due to wear.
