It looks as though that shackle has lost a third of it's section where it met the swivel, due to wear.

That wasn't a hardware failure..it was an operator failure..
It was neglect!
Monel is more corrosion resistant than stainless particularly in the absence of oxygen

Yes thread so corroded that the pin just slipped out
