The boat was an Amel with a electric winch with a foot switch. A woman was hoisting someone with self tailer. The person on the chair had serious injuries when the winch pulled him up through the spreaders/rigging without stopping. Then the woman and a "good Samaritan" were further injured trying to deal with over raps, and a run-away winch while the originally injured guy was being additionally injured.
It was a panic situation that went badly.
There are thousands of these devices around, it was a very rare condition -- but it's an example of the person down below and the person on the end of the bosun's chair not both thinking about what can happen if something starts to go wrong.
Like I said, I'm cautious and I have the person at the winch prepared. We have two stoppers on the main (because of it's size) so someone doesn't have to use the self tailer to raise someone. Then if something happens to the winch, with a flick of the wrist, they can release the line from the winch pulling further. As I'm going up, I'm thinking of the affect of an overrun whenever I'm in a position where I'm close to having my body is a situation where I'd have to push-off.
