What did he not understand about reduncancy?

I agree. There seems to be s distinct lack of redundancy in his prep of the boat. He must have had a generator if he intended to use his charger/inverter to recharge the batteries, but did he not have ANY back up means (solar, wind generator, engine alternator, etc?) ready to go? As to the fuel starvation problems, that's why we carry extra filters. In a pinch, it's not hard to clean the old ones if you have to make them usable again. Someone else already mentioned that his navigation difficulties seem to indicate having any handheld GPS's aboard. One of these and a paper chart would have eliminated the difficulties of getting in through the reefs.

50 miles from your destination and you set off the Epirb? Really? I've no experience around the Bahamas (all my sins are in the Pacific), but is there no reasonable approach to the island, even if it's not from the NE?

Granted, the seas had been bouncing them around pretty good for several days, and you've got to assume this resulted in some sleep deprivation and thus poor decision making, but if what we read is accurate, some exceptionally poor decisions were made both in the preparation and execution phases of this journey.

The lack of redundancy in preparing the boat seems so egregious to me that I've got to assume there are significant pieces to the story that have either not been reported or have been misreported.
