It would not be displayed if the CP was zoomed-out (to display tens of miles in one screen) to set the route. Another thing that could have happened is maybe someone "grabbed" the route or destination waypoint and dragged it accidentally. My experience with the rocker-style mouse controls on CP's is that they leave a lot to be desired, in terms of usability.
We'll probably never know for sure.

- I a 2 mile island doesn't display on your chart plotter using any reasonable scale in that context...

- Every chartplotter I know of has a split screen.
