One time stands out when sailing with friends across the Sea of Cortez. Wife Susie and friend Ruth were managing the cockpit while I went forward to put in the third reef. I felt like I might stream out like a flag at any time while fighting the substantial heel and strong wind. It was slow work but it was comforting to know that I had the short tether clipped to a strong point on the mast. It was at night (of course!) so that adds 30%, perceived wind and internal inclinometer readings and of course the fear factor!
With high lifelines, you still have the option of using a long tether and staying low.
