I am speaking only about user human error - certainly an error in the GPS system, a fault in the AP wiring, a chart error, a misprint in the sight reduction tables (that one is for the luddites), etc could not be considered an error of the user.
Name me a system, or integration of systems, that actually exacerbates user errors by design. Don't give me examples of stupid people or bad installations - just systems that by design cause capable and experienced navigators to suddenly go flying off into rocks, etc.
And give up the "addictive solution" argument. That one is a slippery slope. You wouldn't even be out cruising without gear that was previously described in fear using the above words. Roller furling, GPS, RIB, electronics in general, toilets, etc.
And once again, that was a professional skipper on a well-found boat with no evidence of the problem caused by electronic sirens. Crap just happens sometimes.
